I feel your pain. Literally. I am now having similar pain in my left buttocks, after having majot lower back pain in my right lower back. My pain seems to have shifted to the other side, so i take that as a good sign. (cross my fingers)For what is is worth, a friend of mine had surgery last year for your exact syptoms for years and literally jumped off the table when completed. I am also a soccer player and it is killing me not playing now for three months. The exercises with the video do help so I hope you feel releif.

I have no solutions yet, but it helps to know that I am not alone and not crazy and making up symptoms. My MD looked at me as if I was crazy when I fisrt presented with the low back pain and butt spasm and tingling in my right leg. That was in May,2005. Now I have been in pain and having the most horrible butt cramps/spasm and leg pain(my right leg)that you can imagine. I was reffered to a Chiropractor and was given rx for flexiril, Motrin 800 and a host of others and still progressing with the same pain. So far I am learning that I really have a high tolerance for pain. I will not stop trying to find a solution or cure for this problem, but I will stop running up medical bills until some one can give me a definite treatment and not just more pain meds***jb

Piriformis is a tricky one to diagnos. You can stretch this muscle (lots of stuff on internet) and see if that helps. Make sure you hold teh stretch for at least 25-30 seconds.
It could be referred pain from you back. Has anyone done a passive starught leg test in you to see if there is any nerve involvment?

Re: pain in right butt and down the leg
I have never seen symptoms attributed to piriformis syndrome cause bilateral LE symptoms. I would suspect some of the bodybuilding activities may aggravate the situation or even cause some of it. So xrays and MRIs are both relatively negative in a 27 y.o.....Sacroiliac joint problems are certainly a possibility. The symptoms being bilateral is a significant problem particurily if equal intensity as this can often signify spinal chord pathology.
No changes in bowel and bladder function?
No unexpected changes in weight over the last 6 months?
I would back off the bodybuilding which just causes the extremities to get stronger in relation to the midscetion and start focusing on key midsection muscles when appropriate (meaning when your pain is controllable again).
Here are some key muscles to look up: transverse abdominis, multifidus, external oblqiue.

Hardy,
I am having the exact problem. My pain is in the right butt and down the right leg. It is constant. It is horrible. My doctor perscribed me Celebrex too. It is an anti-inflammatory to reduce the inflammation of my sciatic nerve. I don't want to take it either because it was one of the drugs on the news awhile back that was causing heart problems among other pretty severe things and they were suppossed to stop giving perscriptions out, so I am a little nervous, seems like inflammation may be better than side effects of drug.
Your story sounds very similar to mine. I woke up one morning and couldn't walk. I got on the phone and called Chiropractors trying to find one that could take me in right away. I got better after about a month of chiropractic treatment and I got right back to an active lifestyle. I jogged, Taebo, gazelle, rollerblading and then one day I woke up with the pain again only this time it isn't getting better with any treatments and it seems like all I do is go to appointments for chiropractic, massage, physical therapy, doctors and on and on.
Have you had another MRI or Xray since the pain started back up? I found out I have a herniated disk in the L4-L5 and L5-S1. At least now I know and I can move forward with the right kind of treatment as soon as I find out what that is. I would get another MRI if you haven't had one lately, I wish you well.

I sleep very well at night but I'm in agony in the morning, many tears have been shed first thing in the morning. Looking for an answer as well. I've had this pain for the past 3 years & it's getting worse.

I have had this problem for over 12 years I finaly went to the Doc thinking it was in the hip but when he pressed a muscle in my butt just above the hip halfway from the side to the very backsode of the butt I nearly came off the table... It goes away after a few days transfers from left butt to right butt causes me limping and heavy pain when sitting or laying down... It used to happen once in awhile but now is everyday! Doc gave me two meds IBprofin and and something for the nerve called Neurontin (Gabapentin)This SO much helped for about 2 weeks but now does not help much... He wants me to go in for a steroid shot right into the muscle... we shall see what this does. But I can tell you the pain is extraordenary and I have a Extreemly high pain threshold the pain raidiats down the outside of the leg to knee then goes to inside and at times while walking my leg has gone out from under me to the point of near falling with a shooting shock pain... You know the pain is bad when ya wonder if death is better.... No I dont beleave it is but I wish it would stop hurting....

HEY! I have this pain also! I thought I was just making a big deal out of nothing. I thought I pulled my sciatic this June 2005. The pain is in my right hip and goes allthe way down to my toes. My toes constantly tingle and my leg constantly feels cold. The pain is so excruciating that I can't sleep at night. I go to bed @ 10 and get up @ 2 in so much pain that I can hardly move. The Doc tried to solve this with a sleeping med (Lunesta). DUH.. that didn't work! Then he told me that my hip wasn't causing the pain, it was my ankle and I supposedly sprained it. So he prescribed an air cast. DUH..that just made the pain worse. So now I changed doctors. I saw him last wed. He is having me come in tomorrow to get 2 shots in my hip. I believe he said something about it being a steroid. It is suppose to calm the nerves and relax the muscles and HOPEFULLY make the pain GO AWAY!! I'll let you know how it goes. I had this pain in 2003 also and my old doc gave me Bextra but it didn't help. I have seriously been wondering if I'd be better off to amputate up to my hip Hope someone solves this soon.

Yep me too. Disc bulges at L4 & L5 and butt pain in left, somtimes right butt, occassionally into legs.
Two MRI scans, loads of chiro, physio, osteopathic, massage, even the lose the back pain video.....
still knackered!! although maybe 1% better each month, maybe? or maybe you just get used to the pain and living with it. I have had it constant for over a year now.

Re: pain in right butt and down the leg
Hi Hardy
I too have the same symptoms. It started approximately 6 years ago by a pain in the groin of my left leg. It got progressively worse over a long period of time. About 6 months ago it switched into my right leg and has been going back and forth since then. I also suffer from fibromyalgia and am on pain meds for that, but the pain in my lower back and one or the other of my legs never goes away. I go for injections for my fibromyalgia every week, and in the beginning, the shots in my lower back helped somewhat, but recently even that didn't help. I started falling every now and then without warning and even my walker didn't stop that. Over the past 2 weeks the falling became much worse so my doctor sent me for a ctscan of my lower back and the results were that I have arthritis in my spine, and bulging discs at L4 and L5 which are pressing on the nerves and that is what is causing the radiating pain in my butt and down my legs. Also the tingling feeling, etc. I now have to go and see a neurosurgeon which will probably be months from now as in Canada it takes sometimes forever to get in and see a specialist. I live in Oshawa, but have to go to Toronto to see this specialist. I am attempting to get approval for a scooter through my disability and am waiting a call for a mobility assessment. I am afraid to go out alone because last week I fell and 3 people walked by me without helping me. I will let you know what the specialist says, but that will probably be down the road a bit.

I know what you're going through - Last year I awoke to pain in my lower back and right butt/leg. After being in excrutiating pain for several days I final made an appointment with a Spine Specialist. Luckily I didn't have any damaged disk, but they thought I pinched my Sciatic Nerve. I was referred to a Pain Management specialist and after lots of Physical Therapy, a couple rounds of SI joint injections with Trigger point injections I was free of pain - took almost 3 months.
Then in March of this year I was rear-ended and the pain was back. I have had 2 more SI joint injections and 2 rounds of trigger point injections as well as 2 rounds of PT. Then they did a nerve block, which worked great for about a week and a half. Now they are recommending a Nerve Ablation - I'm scheduled for a 2nd opinion on Monday. There is something, forgive the pun, un-nerving about having your nerves "burned".
I'll let you know how it goes. The med regime they have me on is Motrin 800 mg; Ultram and Zanaflex. I can only handle the Ultram and Zanaflex at night. Alternating heat and cold packs helps.
I just wish I could get rid of the pain in my butt. Feels like I'm trying to balance on someones thumb.

HEY! MY MRI showed that my L5 is bulging in and to the right. The Doc sent me for therapy. I am now in traction three times a week and doing special excercises. I feel it is working some, hopefully it will get alot better. I am on no meds for my pain right now. I just have to watch how I bend, lift, sit, and lay. I always have to remember to use a rolled towel or pillow behind my back and I also have to remember to hold in my stomach muscles. Hopefully on the right track!

Amanda,
What was the result or affects of the traction? Also, how was it performed?
As far as injections go, there's no possible way they can eliminate the problem and there's only a slight chance they will ease the pain...
Think about it this way...
How can an injection correct a life time of physical stress, wear and tear and muscle imbalances?

Re: pain in right butt and down the leg
My pain started a year and a half ago. When I went to the hospital I was treated like I was making everything up because they couldn't prove that I was in pain. All the exams, x-rays, and mris showed nothing. They gave me alot of pain and nerve medicine that only sometimes dulled the pain. Ativan was ok as well as steriods plus flexeril helped and with a few weeks of chiropractic treatment I was better for about 9 months. When it came back there was no way I could afford $50 dollar treatments and I have no way of getting health service even if I thought it could help. Now for me the pain starts like a pulled right groin and then that area just gets worse until it becomes not worth living through anymore. After a while my right leg will start spasming all the way down and that becomes painful too. After the incredible debt from my last hospital visit (medicare lied and said they'd pay for it) I had no other choice but to turn to heroin. Amazingly, I ingested an amount of herion almost too small to see and the pain was IMMEDIATELY gone and hasn't come back since (and I've only done it that one time). The only thing I have now is sometimes a muscle in my lower left hip will spasm for a few hours in the morning after I get too much leg exercise. My chiropractors diagnosis was weak tendons.

I'm having the same kind of pain lately. I had a riding injury last year from a horse bucking and 'bumped' my lower back. I still hurt in lower left back just above pelvis and in the left butt. Just this past week I have been getting more pain (almost like a spasm starting) in the low back and butt and going from the groin to the outside of the leg and thigh to the knee. Then it stops there. My chiropractor says its not the sciatic nerve because that goes down the BACK of the leg. If I twist or turn at the waist too fast it's a shooting pain. I have stenosis at L4 & L5 and degenerative disc disease. I think this is all muscular pain though. I can't get any straight answers either so not sure how I should be treating this. Have done physio, massage therapy, chiro, back exercises and stretches and still have pain every day after 1 year.

Jenny,
I went to see an orthopedic surgeon and more or les got told that this will eventually go away and if it doesn't, to have another MRI done. I was also told I have some fibromyalgia. I take vicodin on a day to day basis only as needed and found that the more active I am during the day, the pain starts to subside. I also have done about everything you can think of except surgery. Chiro was my first choice, then anti inflammitorys, MRI , more anti inflams, physical therapy, epidural steroid injections , more drugs , orthopedic surgeon, now just vicodin and excercise. The tingling down to my toes has gone away and I have started to realize the ankle pain that is due to the back pain is also going away. I am still very stiff in the mornings and feel most of the pain in my lower right back to hip but it is getting better.

I have a stiff lower back with some pain in my right butt.Fortunately the situation is manageable and I do exercises every day for the FTP condition from LTBP program. X rays show that I have compression in the last two vertebra in my lower back. I have not had an MRI scan. I have been doing the exercises for about two weeks now and they definately have improved my condition. This was after abandoning another excercise program which was getting me nowhere.
I feel that I got this injury from 4 years of hillwalking where there is compression on your spine from the uneven nature of the terrian.
Now if I do only a small amount of hillwalking I get a stiff lower back the next day.
It seems to me that even if I feel okay that I cannot undertake any activity which compresses my spine.
Appreciate any comments on my condition.
I would also like to compliment the LTBP team for running a great message board.

Get this, in May I had another MRI. MY L5 S1 was still the problem but was bulging and pushing on the nerve root. I went to see a spine specialist and was told to take Cosamin DS (you can buy at Wal-Mart), Fish Oil with Omega 3 Fatty Acids 1200mg, and 2 Aleve twice a day. In about 6 days I was feeling so much better!! I'm back to grooming dogs, and also seeing a manipulation therapist every 2 weeks. I am for the most part pain free. Just a litle stiff at the end of the day.

I had a really bad problem with pain going from my butt down into the calve of my leg, it got so bad i could hardly walk,I was actually afraid I was going to get down, I had it for six months, finally went to the doctor he ex-rayed it said he couldn't see anything told me to keep taking my celebrex. I had been taking it for joint pain. One day when I was hurting real bad in the morning, I was bending down to put shoes on my client, noticed the pain stopped, so when i started walking again it started back up again, I squatted down again for about a minute, it stopped again so all day long that day everytime it started hurting I squatted down counting off 60 seconds, the pain got further and further apart i kept this up for several days and the pain went completely away, all except of some super sensitiveness in my right hip, which bothers me off and on when riding in a car or when sleeping at night have to sleep on left hip all the time. Tried making my self sleep on my back and them my toes started getting really sensitive and num, and irratating. Now I am hunting for a solution for the sensitiveness, slightly painfulness in my hip. My daughter started having pain in her butt down her leg to which was really painful for her she couldn't hardly walk, I told her what i did she tried it and it went away. Don't know if this will help anyone or not but sure helped me. i am considering haveing a MRi done to see what is causing this, would like to know if there is much luck with an MRI. I went to sevral differnt chiropractors and they didn't help me with this pain.

I went to a chiropracter, osteopath, healer,

The only way to control this is to:
1. Take yourself seriously, (people don't
understand back pain, because it's invisible).
2. Compromise intitially (Your'e Allowed).
3. Fish oils, Carrot,celery,1 apple, juiced (lots).
Swimming (front crawl only). If you can, sauna.
4. Stretch everyday, (compromise your time)
5. Lots of water, Have hope.

I just received the Lose the Back Pain DVD and manual three days ago and for the first time I have real hope that my piriformis syndrome will be healed. The pain has already lessened quite a bit and I am very grateful. I'm realizing through doing this program of exercises more than ever how everything in the body is connected and one thing significantly impacts another.
The question I have is this: on one of my visits to the chiropracter, she showed me the x-ray of my neck. It looked as straight as a ruler and she said that it was actually just beginning to bow out in the OPPOSITE direction then it should. Has anyone else seen an x-ray like that? I wonder if that has made an impact on the pain in my hip [I'm sure it has] and what I should do.

I find it amusing when I constantly read everyone saying stretch stretch stretch for their back pain.I know it is the basis of the LTBP system.
Hasn't anyone else realised how useless this is. It just doesn't work. Lets face it if stretching worked why would anyone still be sufferring from back pain.
There are a million stretch program that have been around forever and they haven't solved the back pain epidemic so why do people keep backing it? Take yoga for example it has been around for 5000 years and that doesn't fix back pain it manages it at best if you have a healthy body.
All stretching does is pushes the body to compensate and any benefit gained from it is either lost when the stretching stops or it eventually runs out of steam and does nothing.
Long term stretching does leave you with sloppy joints which can take the pull from a muscle that might be causing back pain but try going out and doing a vigorous workout or run or play a game of sport and see how long the back pain stays away.
I only know of 3 people who actualy bought the LTBP system and none of them got any benefit so i remain a sceptic.
Everyone posting on this site except a few stooges seem to be just starting out with the program and haven't got rid of their pain yet. Good luck but it is wishful thinking from my perspective.
Those who are supposedly all fixed you gotta ask what they are still doing hanging around this place, haven't they got a life? Stooges perhaps??

Hello
I am not in full agreement with you, but I do have to say that I was doing so much better before I started doing the LTBP stretches. In fact, after a short stretching program, and I wasn't pushing it pass any pain, I had a major flare up that is still keeping me down. I too am becoming skeptical as to the benefits of stretching. I believe mild stretching still has its place to keep things loose. However, I think that some of the stretches in the LTBP program are too much for sciatica sufferers.
I too would like to know of anyone out there that has had a similar result as mine. This is an open forum right???

Hi Sceptic,
What I personally find more amusing is people that just don't understand the true concepts of stretching. You could say the same for people who go on diets, How many people diet and don't lose any weight??
The difference is CONSISTENCY & MOTIVATION!! That is what makes the difference between why it works for some and not others. The reason why I am still helping is just that, I want to continue to help others, you can stand there all day and make accusations about why it doesn't work, but its not all about the stretching, its also the strengthening of the weak muscles. The stretching will only bring temporary relief, but its the strengthening that will bring long lasting relief. And where on earth did you read that Long term stretching leaves you with sloppy joints? The only reason why that would happen is if you did not strengthen as well and what kind of fool would just stretch and not strengthen!!
Also as it has been said by Steve and Jesse it is ensuring that you are stretching the correct muscles. Everywhere you read about back pain it says, strengthen your abs and stretch your hamstrings. Well guess what Sceptic? That don't work for me!
You say you only know 3 people who brought it? Did they try it or did they just buy it and hope that somehow by watching it and trying a few of the stretches here and there that they would feel better.
Hey I have 3 friends that have purchased Dumbells and Barbells over the past year but none of them have gotten any more muscular! Why? Those 2 very special words.
And the second reason why I still hang around on these boards is because I have a 9-5 temporary job at the moment and to know that I can help someone even if it is just chipping in my 2 cents that makes me feel good to know that I am helping others. Just call me human!

Re: pain in right butt and down the leg
up till six weeks ago I was leading a normal life of gentle walking around the shops, gentle cycling, gentle swimming (breast stroke) and some weight training at sports centre, nothing heavy.Been doing the weights and cycling for two years.six weeks ago, i felt a dull pain on the inside of my thighof the right leg whilst swimming.two days later, couldnt swim due to this dull pain. The next day, I had sharp shooting pains from the top of my left leg, that went down the side of the knee and half way down my shin, also shooting pain across the top of my thigh to the kneecap.sometimes have a dull pain in right buttock. leg is very stiff in morning and evening.cant put too much weight on right leg.feels as if it could give way or have that shooting pain again.now, six weeks since this all started, i cn put some weight on the leg,not much tho.most of the shooting pain has gone.very difficult walking, can only do so with a stick.no pain at night, i sleep on my back or side.there seems tobe asharp pain in groin area and still have the dull, now shooting pain in the inside thigh.my GP says, I have sciatica down my right leg and should balance out eventually with exercise, gently though.he told me to contact thePhysio, which Idid andthey seemed to thinkI have a little wear & tare on my hip. they sent me a diagram of some exercises todo everyday.ifthe condition doesnt improve after 3 weeks then im to go back to them.im self employed gardener,48 years old and havent had any back pain. i havent been able towork for 6 weeks and evennow dont feel confident enough.the problem is not bending, the problem is walking...I resemble a penguin! Im not over weight, only 14 stone. heat seems to help. go to bed with hot water bottles.managed to get on my bike once for a gentle ride,my leg killed afterwards.itssooo frustrating not being mobile.I feel so useless, cant walk, cant work...life is not worth living if this continues.

Re: pain in right butt and down the leg
If you go back and read everything that has been done to me, you will see that almost everything that you could think of has been done except for surgery. Well, ever since I was put on the 2 Cosamin DS, Fish Oil w/ Omega 3 fatty acids, and two Aleve twice a day, I have felt sooooo much better!!!! Since about Sept. I am down to 1 Cosamin DS, 1 Fish Oil w/o3fa, and no Aleve unless I need it. I only take the Aleve if I have been lifting things wrong or it seems like when the weather changes. It feels great to be able to do everything I used to!!

Re: Re: pain in right butt and down the leg
Hi Ian,
When i read your last sentence it was like jumping back in time for me as thats how i felt when living a very restricted life in constant pain with sciatica in both legs and back pain. It is horrible, i couldnt walk well either and i wondered where my life had gone, but i found there is hope and i gradually got better. Now i cant advise you medically, your condition may be different. i have just been there with backpain and can only go on my experience. There may be different solutions for different people but it is important to tackle the root cause of your pain as well as the symptoms. You may want to consider if you have muscle imbalances contributing to your pain. Please dont ever feel its hopeless...There are a lot of good people on here who will try to help.
I can share a few practical suggestions from my experience, if any use to you. Always wise to check out if safe for you with your doctor first.
Consider stopping doing things like biking for now, if you have muscle imbalances it could make things worse for you, and you sound in a lot of pain. Instead consider getting in the pool , not to swim, try just floating, treading water, water walking in different directions, scissoring your legs..just being in the water all very gently and only as you feel OK with it.
A hot tub/warm bath /shower may help with pain.Heat on your back, hot water bottles or heat pads are useful.
Try any exercises after a warm bath or shower so you are warmed up. Only go to your tolerance.
Drink plenty of water throughout the day. Ensure you have good nutrition.
Have you considered the lose the back pain system? In brief it is a series of targetted exercises to address any muscle imbalances you may have. You do self assessment and follow the corrective exercises and they support you as you need it.Click return to website to learn about it and there are good educational articles there as well. You can e mail the staff at support@losethebackpain.com if you have questions. It was that system combined with water therapy which gave me relief from pain. I had tried so many other treatments before that, but they were not right for me and did not find those generic exercises the physio gave me helped much at all. You need to judge how you go with them for yourself.
I found i needed to learn as much as possible about my condition, do research and ask lots of questions. Not easy when you are in pain but i felt the better informed i was the more control i had over my care and treatment. Spine health is good for info. Also unless it is necessary and medically urgent dont rush into surgery, there are a lot of questions to ask and options to try first.
Anyway just my experiences if that helps. Sorry for long post. Try to keep your spirits up, be gentle on yourself and let us know how you go.

Re: pain in right butt and down the leg
I would just like to update on my last post of Apr 5th 2006.Since that time I injured my back on two more occasions as a result of playing tennis. After my last injury in March 2007 my back was so stiff that I needed a walking stick to walk around my own house. My recovery from this last injury was never satisfactory. I tried Physio, Acupuncture, various exercises including LTBP,Pilates and Aerobics. After about 2 months the best I could do was a half hour walk around the block. At this stage I had a stiff lower back plus Sciatica.
As a last resort I decided to try Chiropractic treatment.I did require a lot of treatment but at the end of it now I am back playing Tennis and Hillwalking.Yesterday I did a tough hillwalk and I am fine today.I am 64 years of age.
I have seen a lot of chiropractic bashing on these boards maybe some of it is justified. My experience however has been somewhat different although I can only relate to my personal circumstances.
I still do exercises from LTBP everyday I think this is very important, also I have introduced a lot of vegetables into my diet which I do believe have helped me.

John
I'm very glad that you're feeling better.
You raise an interesting point.
Last year I was left completely crippled by a chiropractor, yet another chiropractor was able to help me so much that a degenerated, calcified disk is now improving greatly.
They are not all good, and they are not all bad. Unfortunately, the bad ones can do serious damage.
After I was left crippled, neither he nor any of the other doctors or chiropractors could figure out what had been done to me.
I had to buy medical books and figure by myself what he had done.
One thing that I will say though is that if you have vertebrae which are out of alignment, or a dislocated sacro-iliac joint, then there is almost no way that stretching or anything else will cause those bones to go back into place. A proper series of adjustments will be required.
Any Chiropractor who says that he doesn't believe in Physical Therapy should be avoided like the plague.
Any Physical Therapist who says he doesn't believe in Chiropractic should also be avoided like the plague.
Sometimes a person only needs stretching exercises to correct the problems causing their pain.
Sometimes a person only chiropractic adjustments to correct their pain.
Others [ like you and I need both ].
But, even if a person gets pain relief from only using chiropractic, they have to ask themselves why an adjustment was necessary in the first instance. The reason is [ barring traumatic accidents, certain disk pathologies and a few other things ]in most cases that muscles caused the bones to be put out of place. That type of person should perform exercises to address the muscle imbalances that caused it in the first place.
Unfortunately, many unscrupulous chiropractors and PT's continue taking money from patients when the patients' progress is limited or zero.
When you meet with a chiro for long term treatment, set down agreed goals and timelines. If he does not meet them, go elsewhere.
My wife and I get x-rays performed [ low level radiation, very safe ] every 3 months, and can see the improvements to our disks and spines.

Dave,
I take onboard everything you say.I am sorry to hear your story about the chiropractor, hopefully you are getting better now.
I posted my message because I thought it might just help somebody, not a lot of people post messages with regard to their recovery or partial recovery from injury. In this respect it was interesting to get a message from Amanda.
I am not advocating that chiropractic treatment is suitable for everybody. The decision as whether to accept treatment has to be made by the person involved. In my case the bottom line was that if my back got worse then I probably could recover to my condition prior to chiropractic treatment by means of exercises.
My experience is that you should never give up, keep looking for new ideas and manage your injury as best you can.
